This mobile web application was used by study coordinators at over 25 sites across the country to monitor patient pain.  It allows a patient enrolled in the study to complete a survey tracking his/her pain twice per day using the Data Warehouse Consultants Survey Engine framework.  This application features two portals – one for staff and one for patients.  We have provided a link to each of the live demos below for you to review.  Be sure to note the login information required to access the demo sites.

Purpose:  Allows patients to log in and submit pain surveys multiple times per day.  (Login is DHS04/DHSTest1)
